Vaccines:
We are pro vaccine. We continue to follow the previous, science-based vaccine recommendations and have made no changes to our vaccine schedule.
We were honored May 7, 2026 by the Indiana Immunization Coalition with an award for achievements in vaccinating the children of our community.

Forms & resources
Download the forms you need below. Please complete your child’s name, date of birth, allergies, and other information before sending forms through the patient portal. Forms are free and are generally returned within 24 hours. Please keep well visits and account payments up to date.
For sports and school physicals, send us a portal message—we already have the forms. We can complete them using a well visit from the past year; an additional appointment is not needed.
Head Start & preschool
For daycare, Head Start, or preschool. Complete the family information and leave the immunization page blank—we will attach your child’s vaccine record.
ADHD evaluation · teacher forms
Ask 2–4 teachers, including the homeroom and core subject teachers, to complete these forms before an ADHD or school-performance evaluation. Submit all teacher and parent Vanderbilt forms together through the portal before your appointment.

Each parent should complete a separate form. If only one parent is involved regularly, ask another adult who knows your child well to complete the second form. Independent responses help us understand your child’s behavior in different settings. Submit all parent and teacher forms together through the portal before your appointment.

If you would like us to have your child's previous medical records please have them sent to us on a USB drive. You may also attach them to a message on your FollowMyHealth portal.
We do need Vaccine Records for patient coming from out of state.

These are recipe sheets for increasing the calorie content of breast milk or standard 20 calorie per ounce formula. If you are using a brand other than Enfamil, Similac, or Gerber, please confirm with Dr. Fisher or Caitlynn that these recipes are appropriate!
